This in turn supports social prescribing which connects individuals with community resources and support to improve their health and wellbeing.
Digital Local Connect by MOHT
The Digital Local Connect (DLC) is a transformative digital ecosystem developed by the Ministry of Health Transformation (MOHT) to enhance community-based healthcare coordination. By connecting healthcare providers, social services, and community partners within specific geographical regions, the platform enables seamless coordination of care services and information sharing about patients' needs.
This integrated approach supports care teams in managing patients' health journeys across different care settings, facilitates smoother transitions between various types of care, and leverages geospatial technology to map and connect available healthcare resources with local residents, ultimately strengthening the delivery of community-based care in Singapore.
